Recurrent sarcoidosis after lung transplantation.

چکیده
A 43-year-old man had undergone bilateral lung transplantation 2 years previously for sarcoidosis stage 4 fibrocystic disease. The patient feels well, without significant symptoms, sputa, cough or fever. A control spirometry however showed a highly obstructive, mildly restrictive pulmonary function, with a very low diffusion capacity. Laboratory results revealed a CRP of 6.4 mg/l (nl <= 5 mg/l), ferritin of 669 μg/l (nl 200-400 μg/l) and otherwise normal results. Ferritin is also an acute phase reactant, and in this case a significant elevation indicates an inflammatory state. A high-resolution CT of the lungs was performed, which shows numerous centrilobular micronodules in a perilymphatic distribution pattern (subpleural, peribronchovascular), most pronounced in the upper lungs (axial images A, B and coronal image C). Additional pulmonary biopsy revealed recurrence of sarcoïdosis.

Recurrent sarcoid granulomas in a transplanted lung derive from recipient immune cells.
From 1992-2004, single lung transplantation has been performed in seven patients with end-stage pulmonary sarcoidosis at the Danish National Centre for Lung Transplantation. The objective was to assess whether recurrent sarcoid granulomas in the lung graft are derived from recipient or donor immune cells. Three patients had sarcoid recurrence in the lung graft, but none had clinically overt ext...
متن کاملRecurrence of sarcoidosis following bilateral allogeneic lung transplantation.
We report the first case of recurrent sarcoidosis manifested by clinical symptoms, radiographic abnormalities, and pathologic changes in a patient following sequential double allogeneic lung transplantation. A 40-year-old male patient underwent bilateral allogeneic lung transplantation for end-stage pulmonary sarcoidosis. Thirteen months posttransplantation, he developed fatigue, shortness of b...

BACKGROUND Selected patients with cardiac sarcoidosis undergo heart transplantation, but outcomes may be adversely affected by recurrent cardiac sarcoidosis or progressive extra-cardiac sarcoidosis. OBJECTIVES We present our single-center experience of patients with cardiac sarcoidosis who underwent heart transplantation. METHODS Consecutive patients that underwent heart transplantation bet...

متن کاملLung transplantation in sarcoidosis.
Lung transplantation (LT) is an appropriate therapeutic option for patients with severe, fibrocystic pulmonary sarcoidosis refractory to medical therapy. Survival rates following LT for sarcoidosis are generally comparable to other indications. Timing of transplantation for patients with sarcoidosis is challenging because mortality rates are high (27 to 53%) among sarcoid patients awaiting LT. ...
